Usually, it happens simply by asking the project lead. Nothing more
than a simple request is necessary.
Then the project lead can remove the committer via
portal.eclipse.org > Eclipse Projects > Project Lead For >
[project] > view > [manage] inactive committers on the project
> [committer] > remove.
