Regarding the first email from Ian, I'd like to expose my opinion: I
will be happy the day when Mylar can organize my tasks without I even
notice it is doing that ;-)

For the "oops, I should have created a task for the bug I just fixed"
case, one idea maybe that, when there is no task activated, any
context information be recorded in a "anonymous task", giving the
option to the user to turn it into a "real" task later.
