This is something that needs to be fixed. It was not possible to do it in the manner we wanted in the time that we had, so we still need to reconcile this. Thanks for reminding me about it. Extensions, in particular syntax extensions like custom blocks, are very much a part of AsciiDoc. (The APIs themselves may not be, but that is something that will be discussed/decided/resolved in the spec process).
