In the New Visual Class Wizard, select Other as the visual class you'd
like to extend, then press the Browse... button next to the Superclass
field. Enter JDialog, then press Finish.

This should work ok. We've had a few problems recently with extending
JDialog. If the resulting code has problems, if you fix the code
yourself it should work. Otherwise, please try out the forthcoming M2
build.
