We are configuring a text editor to provide content assist in the usual
way. Rather than just provide a list box of proposals we would like to
instead offer a tree of proposals. A brief look suggests doing this
would involve breaking internal API. How should we best go about this?

This is currently not possible unless implement the content assistant on
your own.
