Toolbar contributions not shown in Customize Perspective dialog [message #59253]

I've added a new toolbar contribution using menu extension point.
Its showing in the toolbar and works ok. But I cannot see it in
Customize Perspective dialog (for example to hide it).
Also another question is, how can I create groups like they are shown in
Customize perspctive dialog?
