I have added a <dynamic> entry to a main toolbar. The implementing class
gets constructed, but its #getContributionItems() never gets called. Are
dynamic contributions not supported in the main toolbar?

Dynamic entries are not supported in any of the ToolBars,
CompoundContributionItem only implements fill(Menu).

It's still do-able with your own implementation (and some hackery), but
the flickering could potentially be a problem (changing the main coolbar
can cause a full re-layout, and the same with view toolbars).
